Your First Visit: We look forward to serving your health care needs. Please arrive 10 minutes before your scheduled appointment to complete paperwork. Please bring your insurance card and health information such as current medications/supplements, immunization record, or copies of recent lab tests.

Discounts: We offer a 20% discount for patients paying in full by cash or check at the time of service. We offer a 50% discount to Bastyr students or those experiencing extreme financial hardship. Documentation of hardship/student status and full payment by cash or check at time of service is required.
